SUBJECT OFFERINGS FOR 2ND SEMESTER, S.Y. 2015-2016
IMPORTANT:
- During enrollment, please come in complete school uniform and bring your School ID and grades for the 1st semester.
- Regular students don't need to undergo enrollment advising if they shall be enrolling in the subjects already plotted for their respective courses and year levels.
- Regular students with irregularities in their schedule (e.g., shiftees, transferees, with previously failed units/ subjects) should undergo advising prior to enrollment.
- Education students, whether regular or irregular, are required to undergo enrollment advising with the Dean of Education.
- All irregular students are required to undergo enrollment advising with their respective deans/ program heads.

Enrollment by proxy shall only be allowed after the scheduled students have enrolled. Please present the following:
- Approved Authorization Letter
- School ID of Student being Proxied for
- Approved Enrollment Advising Form
- If enrolled by proxy, the student shall not be allowed to add or change subjects.
- Enrollees shall be accommodated on a first-come, first-served basis. When you arrive for enrollment, get a number, and proceed to the waiting area at the lobby.
ENROLLMENT PROCEDURE: REGULAR STUDENTS
Schedule of Enrollment for Regular Students
Step 1: SEC FEE
Present your completed Student Information Sheet (to be distributed at the waiting area),
pay the SEC fee (P 200.00), and get a number stub.
Step 2: REGISTRATION
Submit the Student Information Sheet and number stub to the assigned personnel.
The Student Information Sheet shall be distributed at the waiting area.
Please fill it up properly and completely.
Step 3: ENCODING
Confirm subjects with the assigned personnel.
Step 4: ASSESSMENT
Claim your assessment form.
Step 5: PAYMENT
Proceed to the cashier’s office for payment.
Step 7: REGISTRATION FORM PRINTING & ID VALIDATION
Present your Official Receipt and ID at the registrar’s window
for the printing of your Registration Form and validation of your ID.
ENROLLMENT PROCEDURE: IRREGULAR STUDENTS
- If your name does not appear in the enrollment schedule posted, you are considered an irregular student.
- Distribution of stubs for enrollment is on October 19, 2015, 10:30AM, at Room 701. These stubs shall only determine your enrollment schedule: October 27 (Morning), October 27 (Afternoon), October 28 (Morning), October 28 (Afternoon), October 29 (Morning), or October 29 (Afternoon).
- October 27 (Morning) is reserved for 4th year/ graduating irregular students.
- Please bring the following for advising:
- Updated flowchart
- Grades last semester (1st Semester, SY 2015-2016)
- Completed Enrollment Advising Form
Step 1: ADVISING
To avoid long queues and to speed up the enrollment process,
pre-enrollment advising is on October 20 and 21, 2015 (9:30AM to 4:30PM).

and pay the SEC fee (P200.00).
The Student Information Sheet shall be distributed at the waiting area.
Please fill it up properly and completely.
Step 3: REGISTRATION
Submit the Student Information Sheet and number stub to the assigned personnel.
Step 4: ENCODING
Submit your Advising Form to the assigned personnel for the encoding of subjects.
Step 5: ASSESSMENT
Claim your assessment form.
Step 6: PAYMENT
Proceed to the cashier’s office for payment of tuition fees.
Step 7: REGISTRATION FORM PRINTING & ID VALIDATION
Present your Official Receipt and ID at the registrar’s window
for the printing of your Registration Form and validation of your ID.
