2. Please call the Guidance Office at 252 3675 to schedule your final interview and to fill up necessary forms. You can look for Ms. Imee Mendoza.
2. Please submit the following documents to the CKSC-FBAS Registrar's Office before May 19, 2014:
- Honorable Dismissal
- Transcript of Records
- Two photocopies of Birth Certificate (NSO)/ iCard/ Certificate of Naturalization for naturalized citizens
- Two copies of latest 1x1 colored photo
- Certificate of Good Moral Character
- Interview Slip
3. Passers are required to attend the orientation on May 30, 2014 at the 7th floor, CKSC-FBAS.
4. Enrollment is on June 3, 2014.
5. First day of classes is on June 4, 2014.
